UNDER AGE 18 TEEN VOLUNTEER PERMISSION SLIP

Thank you for enabling your child to volunteer for the Religious Education Program this year.

As a volunteer, your teenager will be responsible and instrumental in the success of the program as well as being an inspiration to the children and adult Catechists they support.

Because your student is under the age of 18 and volunteering for a Community Service commitment, it is vital that they attend and follow the rules of conduct expected in all classroom situations. Respect for those in authority and the safety and well-being of all participants are of utmost importance!

The use of cell phones and other devices will not be used during session hours. The adult in charge of the class or group must be aware of where your teen is at all times.

According to the guidelines set by the Archdiocese of Newark and the Protecting God’s Children
program, it is required that your permission for your child (under age 18) must be kept on file
during the duration of their participation.  Please sign the page below and return it to our office as soon as possible to the address above or the first day of participation at the latest.

Any papers or references or proof required by your child’s school or Confirmation program will be happily filled out by me on request.

Blessings to you and your family during the coming year!
